 The Patient Support Service is provided by the four GP practices in the North Buckinghamshire Primary Care Network. 
​We work alongside a wide range medical professionals & local organisations to help people access the services required to keep our patients safer, more independent & happier in their own homes. 
The service is free and open to ALL patients that are registered with one of the surgeries below. If you feel like you or someone you know would benefit from talking to a member of our team
